HAZARD IDENTIFICATION & NEAR MISSES REPORTING FORM  

The Ohoka Rugby Club wish to provide a safe environment for all people attending rugby matches at MSC. To achieve this, we ask all Coaches and Managers to identify any hazards or near misses that may cause harm to players or spectators in and around the MSC grounds.
