No one wants to have an accident when playing softball, but sometimes they do happen.

Please find a copy of the Spartans Accident Report form on this page, print it off, and email it back to the club at spartansoftballclub@gmail.com or if the secretary is available you can hand it to them.

OSA has their own form for Saturday play, but this can be used at practice times or if you prefer to send us the information.  Privacy will be upheld at all times.
