At the MIramar Club we take the Health and Saftey of our students seriously.
Accidents and Emergencys do happen so please make yourself familair with the following.
Copy of the Emergency and Evacuation Plan is below (please note this is not the form that is required to be completed but a copy of the plan that is followed in the event of an emergency)

IF you are either of the following, then ACTION IS REQUIRED
- If you are the Parent or Caregiver of a student under the age of 18 years old your Action is required.
- If you are a student receiving this email and are under the age of 18, please ensure your parent is aware that thier Action is required.

This form is designed to provide the club with the emergency contact details of the 5 people you authorize to collect your child/children in the event of an emergency. Our students can not be released in the event of an emergency without this information.
Your details and data will not be used for any other purposes other than being stored on our internal secure system and as a printed/digital copy in a secure location that can be accessed in the event of an emergency or accident.
