​​​​​​​This form is for Associations to provide information to the National Executive Committee of the NZPF to assist in advertising seminars / clinics for Associatons. The form has a table to report how a seminar / clinic was attended and results. If an Association incurs expenses hosting a seminar / clinic may be refunded at the discretion of the NEC upon completing the reimbursement form, attaching invoices/receipts and sending it to the NZPF Secretary *calendar, report , reimbursement form.
