Referees and Assistant Referees are required to report all incidents that result in a yellow card, red card, referee abuse, serious injury or if a blue card has been issued to a player with suspected concussion.
These forms are required to King Country Rugby Union within 48 hours of completion of the match for judicial purposes. By submitting the online form below King Country Referee Education Officer and the Rugby Manager will receive a copy of the form. They will contact you if your incident requires further information, or if there will be a judicial hearing for your incident.
